About Pi-GST as a Biomarker

Pi glutathione s-transferase (πGST,piGST, GSTpi) is located in the distal tubules of the human kidney whereas alpha GST (αGST) is confined mainly to the proximal tubules. πGST is released into the urine of normal individuals and any event which causes distal tubular damage may cause increased release of pi GST into urine. Elevated urinary pi-GST levels have been shown to be indicative of distal tubule damage in renal transplant rejection nephrotoxicity, infection, diabetes and chronic renal injury.

Since the release of aGST has been shown to be associated with proximal tubular damage, simultaneous measurement of piGST and aGST may allow discrimination between proximal and distal tubular damage.

Pi-GST is the only commercially available urinary biomarker for distal tubule injury.
